How to Get Started

To be considered for surgery, you must go through a screening process. This thorough process is to ensure that bariatric surgery is the best treatment option for your health. The program progression is as follows:

1.       You must fill out an application. To do so, either click or copy/paste the following link into your browser address bar: https://www.surveymonkey.com/s.aspx?sm=OZTNcg5F08MCyQisMs2fiA_3d_3d

2.       Download, print and review the Patient Guidebook

3.       The questionnaire is reviewed to determine eligibility.  It is imperative that you answer all questions completely including contact information about your physicians.  Otherwise, your clinic appointment and surgery date will be delayed.

4.       Once the application is received, you will be notified by telephone.  If a delay is noted, please call the office at (310) 825-7163 to ensure your application was received.

5.       You will be contacted by letter or phone with a consultation appointment date/time, or an explanation describing why you do not meet criteria, and suggested alternatives.

6.       You must attend an information seminar prior to being seen in clinic. Please see calendar of events for schedule.  Seminar attendance can be done without prior submission of an application.  Clinic appointments for new patients are available on Tuesday mornings.

7.       Your consultation appointment with surgeon, dietitian, and bariatric nurse. You are expected to have studied and researched extensively about the surgery prior to your appointment and come prepared to explain to the team about your research, pros/cons of each surgery, and the reasons behindyour choices.  An educated patient is a UCLA Metabolic & Bariatric Surgery Program requirement.

8.       At your consultation appointment, you will be given a pre-operative checklist of items to complete prior to surgery.  You will also be able to register for the mandatory pre-operative education class.

9.       Review the Emmi Program and print a list of questions.

10.   Once a surgery date is scheduled, you will be given a pre-ooperative appointment with the surgeon and possibly with the anesthesiologist.

11.   Surgery is performed at UCLA Ronald Reagan Medical Center, Westwood.

12.   Post-op follow up appointments with the bariatric surgery team.
